Reduce Readmission Risk with AI follow-ups after discharge
Create an AI that follows up with patients after discharge to reduce risk of readmission. About 15% of patients are readmitted within 30 days of a discharge, and there are comorbidities that can greatly increase this risk for curtain patients. It is very costly to readmit patients, and CMS does not like it.
Aligns with our patient engagement focus and AI forward strategy.
Inspired by Tricia Baird MD FAAFP MBA and other Corewell Health leaders work in Epic to follow up with patients after discharge to lower the risk of readmission within 30 days of discharge. Corewell has set up…

Post Care Remote Forms : PROM Surveys
I noticed a healthcare tech vendor advertising they can boost PROM scores, and wonder if we should pursue such as well.
NOVA's tooling that is used for RPI could also be adapted to provide post-care or discharge solution. Much of the tech to do so is already done in NOVA. Its similar to RPI but in reverse order.
This could be remote forms that request the patient fill out surveys like PROM (patent response outcome measurements).

Referral Use Case Support
Twice in the last 3 months we have run into opportunities related to referrals to different types of care in Canada. One is tied to an RFP in Ontario and one is tied to a previous RFP in BC tied to Fraser Health. WE have these RFPs to share but cannot attach files here. Andrew believes there will continue to be these RFPs in the coming year. The Nova Platform has many of the elements needed to address but definitely some Dev work related to scheduling and other features

Lab Requisition Printing
We have a client looking to see if we can ingest orders from meditech and complete a standardized lab requisition form from the Province so they have a paper copy they can give to the patient as well?
It would need to pre-populate with the provider and patient's demographic info as well as all info entered in the lab orders selected, mostly likely passed through an HL7 message from Meditech to populate the form.
Could be used for any EHR once the dev is done on one EHR platform. We would likely need to specify the lab orders themselves or…

Referral Intake
Now that Nova can ingest third party forms (pdfs, faxes, etc.) clients in Canada have a need for a solution to manage inbound faxes/referrrals/orders and send them into their EHR scheduling queues for management by the scheduling team.
Most every health system in Canada has long wait lists for things like CT, MRI, Specialist Referrals, etc. so these inbound referrals/faxes/orders typically go into a queue to be triaged and scheduled by the scheduling team at the hospital.
We would need to extract the patient data from the fax if possible to allow for proper patient matching and potentially encounter/visit creation…

Forms Management to include Non Interlace Forms
City of Hope uses Interlace Health for forms creation in a number of areas. However they use Epic Consents for procedural consents. There may be a 3rd set of forms created by another tool. COH would like any "forms management" module we create to be able to provide some level of tracking (form name, tool used to create it, most recent version/date) for all their forms regardless of where it was created.
1 voteThe favor of form management is on the Nova product roadmap along with some other capabilities that will allow customers to manage their own forms and maintain in Nova without having to export to edit the PDF and re-import.

Print Only Product
Design a product that is geared towards only printing forms and not digitally completing. The only digital information entered would be prefill data. No digital signing.
The product would allow for easy selection of encounter records and selection of forms to print for the selected encounters.1 vote -
Off-Line Downtime Solution (Cyber Security play)
Per feedback from the team and folks I talked to at HIMSS, cyber security is THE top initiative for IT right now. It would nice to have a play for that via providing an off-line downtime solution that doesn't require network availability. Basically WFI and Reg Downtime installed as vm's on workstations (laptops) that can be accessed during a downtime or cyber attack. We basically already have these solutions but there should be some thought to how those solutions would be configured or "solution-ized" so they can be marketed appropriately.
