  1. City of Hope uses Interlace Health for forms creation in a number of areas. However they use Epic Consents for procedural consents. There may be a 3rd set of forms created by another tool. COH would like any "forms management" module we create to be able to provide some level of tracking (form name, tool used to create it, most recent version/date) for all their forms regardless of where it was created.

  2. In Web Form Imprint it would be great if we could set a threshold, which would be different for each customer, to prompt users if they are sure they want to print x documents.

    We have at least one person every day that tries to print hundreds to thousands of forms. In our case we would set it to 18 and if someone selected the entire site it would prompt "are you sure you want to print 651 Forms?" This would greatly save on paper and our helpdesk trying to kill print jobs.

  4. Use Case 1 (Top priority): Physicians would like to sign forms electronically on a personal phone. They would need to have access to view the signature queue and complete signatures.
    Use Case 2 (Second priority): Our staff have company issued phones and we want to use them as a backup to tablet devices when none are available. We want staff to be able to have both patient and staff complete and sign forms on the phone device.
    Use Case 3 (Reach): Home health users would like to use their phones to complete documentation while at a patient home.

  5. A practice as simple as a nurse checking in on a patient for vital signs. Digital rounding replaces pen and paper with electronic forms geared to the task at hand. The forms help ensure information is tracked and communicated and the process is more consistent.

  6. Staff would like the ability to streamline and simplify the patient referral to see a specialist, another specialist or have a specific test conducted. We believe this is a very paper intensive and time consuming process across multiple groups within the health system setting.

  8. EHR systems allow for physicians to define phrases or text they use often and enter in documents using special characters. Physicians are asking for the same functionality in Nova ideally using the same files they already stored this information in. If not possible to use the same file, then allow for importing from a file.

  10. This would be similar to how we share forms with remote patients, but the documents are just informational and do not require the patient to complete. The documents are provided to the patient for them to download and print if needed. This will help facilities provide test results, medical history records, and other similar documents to patients extending our value beyond our forms. These documents would only be available for a limit time frame for patient access and download.

  11. Provide reporting or analytics on demographics and adaption rates of patients who use Remote Patient Intake. I know this is hard to collect because that would be coming from the other systems. Can a report be built in SF that tracks % of forms that get completed in advance and can break it down by patient age groups.
